A Substitute School Nurse shall perform the following:
provide specialized and specific health care pursuant to student’s individual health care plan or individualized education program as determined by a physician’s or advanced practice nurse’s care orders
administer medication as required
complete and maintain documentation of first-aid and administration of medication as required
fill out appropriate accident reports
maintain effective communication with medical, nursing, and district personnel
provide nursing care consistent with school nurse certificate at offsite locations and in the school building
perform other assigned tasks as required by administration.
Substitute nurses that are applying for a County Substitute Certificate need the following: 1. County Application Form (Completed at the Board Office) 2. AppliTrack Application (Completed on-line) 3. Copy of a current and signed NJ Professional Nurse License (Brought to the Board Office) 4. Fingerprinting - Apply on line at www.nj.gov/education/educators/crimhist a. For new applicants, refer to directions on www.state.nj.us.education/educators/crimhist/new b. For archive application requests, go to site in #4 above. Click on "File Authorization & Make an Electronic Payment". Choose "Archive Request". Enter your social security number and fill out the application. Make your payment and print the confirmation page. In 2-4 weeks print your approval notice by going onto the above website. 5. I-9 Form (Completed at Board Office and bring a passport or a driver's license and social security card) 6 Oath of Allegiance (Completed and notarized at Board Office) 7. W-4 (Completed at Board Office) 8. Mantoux (TB) test to be taken by a doctor or drug store clinic and read two days later showing negative results. 9. Bank check or money order in the amount of $125 made out to the "Commissioner of Education". Substittute certificate is good for 5 years.
