General Position Description: Provides comprehensive nursing services to women and their families, starting during pregnancy and throughout infant/toddler development, in the client’s home or other community setting.
Essential Duties and Responsibilities:

Skills/Job Knowledge/Abilities: Able to establish and maintain a team atmosphere of communication and collaboration for all that reach out to the Department. Able to communicate clearly, effectively, with confidentiality, openness and professionalism with families, colleagues, and stakeholders Able to listen attentively and accurately to the client’s current experience. Must be self-directed and be sensitive to cultural and linguistic diversity. Excellent customer service in stressful situations. Maintains a professional appearance, communications, and actions. Basic knowledge of the respective evidenced-based home visitation model’s (MECSH or NFP) policies, practices, and procedures. Knowledge of prenatal and post-partum maternal health; the dynamics of intimate partner violence and safety planning; infant/toddler typical and atypical development; family systems and principles of family-centered care; and community health and social service resources. Possess a high level of computer proficiency.
Working Conditions/Physical Requirements (with or without accommodation): Client in-home and office environment with multiple interruptions in person and through computer and phone. Will work outside and inside environments, as needed. Daily travel to client homes and other locations. Interacts with employees and members of the public on a daily basis. May lift normal office equipment and materials up to 25lbs. Visual acuity to read information from computer screens, forms and to assess the wellbeing of clients and children. Able to speak clearly in conversations and general communications. Hearing ability for communication in persona, phone, and/or other electronic methods. Manual dexterity for typing and writing. Able to stoop, squat, reach, pull, push, stretch, ascend and descend stairs, stand and sit for long periods of time. May be required to work additional hours or days depending on circumstances.
