Responsibilities:

  • Assumes primary responsibility for program participant caseload that includes the assessing, planning, implementing & evaluating phases of the nursing process.
  • Maintains up-to-date records (including annual Restricted Health Care Plans) so that problems, plans, actions & goals are accurately and clearly stated and changes are sent to Pace RN as they occur. Review at least monthly any documents regarding individual’s in the 1:2 Special Needs program e.g., Restricted Health Care Plans, Care Plans, Care Logs, Med logs, etc.
  • Trains, instructs and monitors Pace Staff on any health related Mandatory Training, Restricted Health Care Plans, Care Plans and implementation of appropriate nursing procedures in accordance with CCL regulations and RN input, at least annually.
  • Supervises, provides feedback and documents observations of Pace staff implementation of nursing procedures for those individuals in the medical model and those with Restricted Health Care Plans with RN input.
  • Provides classes and activities at the Pace program sites to program participants when not engaged in medical procedures or other assigned duties.
  • Initiates communication with physician(s), Pace staff members, other individuals and agencies as needed to coordinate optimal care and use of resources for participant/family. Inform RN of any communication and accurately record on nursing phone logs.
  • Maintains regular communication with Executive Director, Program Director/Manager and Pace Registered Nurse to review caseload, care plans and any issues or concerns regarding participant’s needs.
  • Maintains regular communication with participant physician(s), families, care providers & funding agency RN (NBRC) concerning Restricted Health Care and Care Plans. Provides input to the RN, at least annually, to assist in developing and maintaining the Restricted Health Care Plans following CCL and Pace Solano requirements.
  • Meets regularly with Pace staff to review goals & plans for participants in the 1:2 Special Needs Program and participants with Restricted Health Care and Care Plans. Keep in contact with Pace RN for needed training and supervision of any new procedures. Participates in Pace in-service training programs for Pace staff that will be involved with participants in the 1:2 Special Needs Program.
  • Attends Interdisciplinary Team meetings and facilitates discussion of issues of those in the 1:2 Special Needs Program at Pace. Attends all Mandatory Staff Training that may occur after your typical scheduled work hours. Report all suspected abuse in accordance with Mandated Reporter regulations and procedures.
  • Display professional and respectful attitude towards participants, co-workers, supervisors and assignments. Ability to lift/transfer/push/pull/bend/turn/pivot up to 38 lbs. occasionally while assisting participants including those in wheelchairs as a support in daily program activities and personal care needs.
  • Ability to sustain a forward bend occasionally while assisting participants in assembly tasks, table top activities and eating lunch.
  • Follow agency policies and safety procedures on an ongoing basis. Other related duties as assigned.

Schedule:

This is part-time position that will work 25 hours a week, 5 hours per day, 5 days per week (Monday-Friday). Candidates for this position will help assist and cover all program sites throughout Solano County as needed. 


Salary: $30.00 an hour, 5 hours per day, 5 days per week (Monday-Friday).

Requirements

Requirements:

  • Minimum of one year of varied work experience as a LVN, preferably in either an acute or home health setting
  • Possess a current license to practice as a Licensed Vocational Nurse (LVN) in the State of California
  • Must possess a valid CA driver’s license, proof of current car insurance and an acceptable DMV printout
  • Must pass pre-placement physical which includes skin T.B. test or chest x-ray, breath alcohol test (BAT), drug screen, and pre-work screen (back evaluation which includes the ability to lift/transfer/push/pull/bend/turn/pivot up to 38 lbs. occasionally; ability to stoop, turn, reposition, transfer and ambulate participants; requires manual dexterity sufficient to provide hands-on care to the participant under variable conditions and settings; and, ability to sustain a forward bend occasionally
