Cabanda Care Inc is located in Rosewood a regional town located 20km west of Ipswich. Opening in 1987 as a 26-bed hostel, Cabanda was a vision of the local community to provide an aged care facility where people could age in the community they lived in.
Since then, we have grown to include a 71 Bed Residential Aged Care Facility, a retirement village with 42 independent living units, Centre based Respite and Social Care Group, Home and Community Care, Meals on Wheels, Community Transport, and the Cabanda Café.
Cabanda is the largest employer in Rosewood and the vision of Rosewood residents living in a community with those they have known all their lives has been maintained. We are proud to say we are still owned by the local community.

Description

Position Purpose

The Assistant Nurse – Home Care under the supervision of a Registered Nurse and/or Community Care Coordinator provides care based on the clients’ assessed needs in a client’s home promoting quality of life within the boundaries of best practice and competency.

Main Tasks

  • Provides and promotes optimal client care, taking into account physical, psychological, social and spiritual needs

  • Assists clients with personal care and activities of daily living including personal hygiene, showering, assisting with meal preparation, house cleaning, laundry and ironing

  • Provides quality client care according to each client’s individualised care plan

  • Recognises changes in clients health status and reports to the Community Care Coordinator and/or Registered Nurse

  • Provides accurate and comprehensive documentation

  • Encourages and promotes independence in activities of living as per the clients individualised care plan

  • Observes and protects the client’s right to privacy, dignity and respect

  • Supports clients to participate in community based social activities and recreation

  • Safely transports clients and carers as required

  • Assist carers by providing respite from their role, which may include in home respite and overnight respite

  • Assist clients with medication within scope of practice, where delegated and skill competency has been attained

 



Skills And Experiences

Qualifications

  • Certificate III in Aged Care (or accepted equivalent)

  • Qualification in Medicine Assist (desirable)

Experience

  • Previous experience in a Home Care setting or Aged Care is an advantage

  • Efficient and effective organising skills

  • Effective oral, written and interpersonal skills

  • Empathetic and caring approach to Cabanda clients

  • Be able to work unsupervised

Knowledge

  • Knowledge of how to maintain and establish effective interpersonal and communication skills with other staff and managers

  • Competent with a range of computer-based reporting systems including smart phones and tablets

  • Has sound knowledge of mechanisms available to staff and Clients for the provision of feedback and takes appropriate action to follow up comments, complaints and compliments
