Job Title:       Clinical Nurse Manager

Location:        On-site at both locations, Washington, DC and Hyattsville, MD  

Classification: Exempt/Salaried

Synopsis: The Clinical Nurse Manger at La Clínica del Pueblo (LCDP). is an integral part of the clinical leadership team. As part of a multidisciplinary team including the Chief Health Officer (CHO), the Director of Clinical Services, and Health Center Managers, the Clinical Nurse Manager is responsible for implementing LCDP’s model of team-based, person and family centered care. This role includes the direct supervision of all clinical nurses.

The Clinical Nurse Manager works collaboratively with the leadership team to ensure that LCDP’s clinical care meets the needs of the LCDP patient population, satisfies external regulatory requirements, and has clear goals to support the delivery of high-quality person and family centered care, including acute illness management, chronic disease management, and preventive care.

Required Education and Experience: 

  • Registered Nurse (RN) license with relevant state licensure
  • Bilingual in English and Spanish
  • Minimum two (2) years’ experience supervising clinical staff in a medical setting
  • Minimum three (3) years’ experience working in community health or patient care setting

Preferred Education and Experience:  

  • Bachelor’s degree in Nursing (BSN)
  • Population health/care management experience
  • One (1) year experience working with Latine communities  
  • One (1) year experience in HIV care
  • Experience in Care Coordination and Transition Management
  • Knowledge of Federally Qualified Health Center (FQHC) compliance and Patient-Center Medical Home (PCMH) accreditation
  • Experience working with clients or patients and staff from diverse socioeconomic, ethnic, and cultural backgrounds

Required Skills/Abilities/Certifications/Licenses: 

  • Current nursing license in both DC and MD
  • Current BLS Certification
  • Strong managerial skills, including personnel supervision
  • Excellent skills in nursing assessment and development of plans of care  
  • Strong critical thinking and analytic skills
  • Ability to effectively evaluate care across the continuum of care
  • Ability to work collaboratively with a diverse interdisciplinary team.
  • Excellent oral and written communication skills.
  • Proficiency in MS Office, including Word, Excel, and PowerPoint and electronic health record systems

Duties and Responsibilities:  

  • Oversee nursing and care coordination, aligned with overall LCDP clinical and organizational goals.
  • Implement, and monitor ambulatory care nursing practice in accordance with regulation and delineated scope of practice.
  • Ensures high quality of care, smooth patient flow and team coordination for patient care.
  • Performs RN clinical care duties in a team-based primary care model.
  • Schedules working hours and assigns responsibilities to provide organized daily activities for the nursing staff and paraprofessionals.
  • Responsible for the administration and application of all clinical nursing functions, procedures, and protocols. 
  • Ensure competency of nursing staff through education, training, and regular evaluation.
  • Collaborate with clinical management team to implement and evaluate population health activities.
  • Participate in Quality Assurance, Standards of Care, Risk Management, and Infection Prevention activities.
  • Participate in quality improvement activities to maintain and improve clinic goals, including quality of care, staff experience and patient experience.
  • Partner closely with quality staff and clinical leadership to maintain and expand patient-centered medical home (PCMH) accreditation.
  • Ensure adherence of nursing team to LCDP’s standard of care, including but not limited to documentation, patient follow-up, care coordination, transition of care, and tracking of referrals and diagnostic procedures.
  • Lead MA Lab and QA Competency Sign-Off- Once all competencies are satisfactorily demonstrated, sign and date the checklist to officially acknowledge the Lead MA's proficiency.
  • Perform other related duties as assigned.

Supervisory Responsibilities:  

  • Directly supervise clinical nurses.
  • Proactively solve problems and manage corrective action, as needed.
  • Recruit, interview, hire, and provide clinical onboarding for relevant staff.
  • Provide timely, appropriate, and ongoing training and development, as well as periodic evaluations and reviews.
  • Supervise training of new hires to ensure completion of competency checklists.
  • Communicate clearly, concisely, and accurately, creating a working environment that provides growth and job satisfaction.
  • Provide constructive and timely feedback to staff.     

Physical Requirements:  

  • Prolonged periods of standing and working on a computer  
  • Must be able to lift up to 30 pounds at times
