We invite you to join our Hematology/Oncology team as a Specialty Nurse Navigator.

As a Specialty Nurse Navigator supporting a malignant hematology oncology MD you will be responsible for:

Ø  Navigating patients through the continuum of care acting as a liaison and resource between the physician, other departments, and the patient.

Ø  Establish specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance.

Ø  Provide in person, clinic visits as needed at the initiation of care and throughout transitions of care throughout all of our hematology/oncology clinic sites, as needed.

Ø  Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.

Ø  Providing disease and treatment specific education tailored to meet patient’s individual goals of care.

Ø  Onsite rounding throughout treatment plan implementation as needed to provide needed in person patient evaluation.

Ø  Provide direct, hands-on nursing care, as needed. 

Ø  Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Assisting patients with social services needs by identifying resources in the community and referring to these programs.

Ø  Collaboration with system wide authorizations team, scheduling in other departments, as needed to ensure patient receives timely and comprehensive care.

Required Education/Experience/Specialized Skills:  

·                     BSN plus 3 years recent clinical nursing experience in a variety of clinical settings.

Required Certification/Registration:

·                     CA RN License

·                     BLS if working in a clinical setting.

·                     ONS Chemotherapy and Biotherapy.

Preferred Education/Experience/Specialized Skills/Certification:

·                     MSN

·                     OCN if working in Hematology ONN-CG if working in Oncology area.

·                     Two years experience managing care of relevant specialized patient population.
