As our Population Health Clinical Nurse you would be responsible for running some of the most exciting and complex population health and disease management programs. To be a successful Population Health Clinical Nurse you will need to be a Registered Nurse with a passion for improving patient outcomes. We need someone with a great desire for working hard to assist patients and improve patient compliance, being hungry for growth and opportunities, always eager to jump in and embrace the challenge.
You will be successful if you can communicate effectively with other members of the team, including our Team Pharmacist and Medical Management Team Lead. Also being able to communicate your Nursing knowledge effectively and can translate the medical terminology from TPAs and other medical vendors.
THE POSITION
Population Health Clinical Nurse Responsibilities
Review and monitor medical data from various TPAs to ensure clinical positive outcomes
Work with Medical Management Team Lead and Team Pharmacist
Reviewing communication strategy to enhance compliance with evidence medical norms
Work with groups and their members to develop strategies for increasing compliance for chronic compliance and preventative compliance conditions
Develop video communications to increase patient outcomes

A successful candidate will have:
· Be a Registered Nurse (RN)
Preferred Qualifications
· Background in interacting with Medical Carriers, TPAs, PBMs
· Experience with corporate wellness, disease management, or population health programs
